Chapter 9 - Unstable

Priya and Prathmesh went to hospital together but Priya told him to go first so others wouldn't get the idea they came together.

Later Priya told everything to Priyanshi and Tista that was happening.

Priyanshi: You can come to me!

Priya: Hey, it's ok. I'm fine and it would be rude of me to leave just after a night when he is giving so much effort to make sure I feel at home.

Tista: Okay got it but how are you feeling at his place?

Priya: There is a strange feeling but I don't know how to describe it.

Tista: How do you feel towards it? I mean the feeling is bad or good?

Priya: It's...I don't hate it.

Priya pauses for a bit.

Priya: I like it. It's good I guess.

Tista: Priya, is it the feeling that you like or is it him whom you like?

Priya: I...am not sure. I don't know.

Priyanshi: Wait what is going on? Tista, why are you questioning her like this?

Priyanshi had no idea what Priya and Tista were talking about. She was clueless about Prathmesh's feelings.

Priya: Ah..it's.

Priya told Priyanshi how Prathmesh had feelings for her and how she was not able to know how to give his feelings back.

Priyanshi: I had no idea of this.

Priya: It's ok.

Tista: I pity him. He is such a fine guy for you.

Priyanshi: But what about her? She is never able to have someone whom she can consider as her lover.

Priya: Let's change the topic.

Priyanshi: OK, fine. So do you have everything to live at his place?

Priya: I need a new pair of clothes, some cosmetics and my snacks. But I don't wanna bother him after all he lives alone and I'm only living for a while.

Priyanshi: Wanna go shopping with us?

Tista: Nice idea.

Priya: Really? But when do we get the time?

Tista: We work from 8 am to 5 pm. 10 hours per day as in India not more than 12 hours of Doctors' work is exceeded.

Priya: I'm working in the ER Department where the doctors have to work 6 hours but it usually is in between 6 hours to 12 hours and sometimes 24 hours of work in the ER.

Priyanshi: ER is a headache I used to see my brother go to in the middle of the night to Hospital because of an emergency.

Tista: I don't know much about ER. Priya tell your experiences.

Priya: Oh okay. The shift of a doctor in the Emergency Department or Emergency Room (ER) can be anywhere between 8 hours to 24 hours.

Now what exactly did we do in this period?

•Gossip

•Have food

•Tea Breaks

•Roam around

Naaah! Those are Luxuries.

So, Let's quickly get to the serious talk.

As soon as a doctor enters the ER-

You find a couple with their 3-year-old child who has been suffering from Acute Diarrhea for the last 3 days and the parents haven't shown the child to any doctor for three bloody days! The child is severely dehydrated with sunken eyes, dried out tongue, and minimal energy to move the limbs. You quickly finish the clinical examination and then, It's Go Time!

As soon as you are done handling the first patient, and are preparing to sit on your chair to write down the notes, enter the second patient who has been found unconscious by her relatives early in the morning. You pray internally that she isn't dead (I am being blunt). You quickly check her Vitals, and call for ECG, while simultaneously taking history from relatives who don't have a clue about her medical history. On further probing, you find that she is a diabetic and check her Random Blood Sugar (RBS) which comes out to be 785! And that is not the lucky number you would want your RBS to be at any given time in your lifetime. Poor Lady had forgotten to take her Insulin Injection last night.

Now after managing these two and finishing their notes, arrives a middle-aged man with severe chest pain, almost crying and wincing with his right hand kept over his central chest. He lies down on the bed. You see his bloodshot red eyes. You ask his relative - Peeta hai kya (Does he drink alcohol). And you get to know about the Weekend Binge Drinking Ritual. They come with the assumption of a heart attack and leave with sadness when it turns out to be just acute dyspepsia (heartburn).

Sometimes you can be unlucky and will have to experience a multi-casualty case. It may be a heavily loaded vehicle that overturned a person, or an old roof that collapsed during a renovation. And street fight injuries or food poisoning in a school. Such cases can be damn challenging and can take on your nerves if you are the lone attendant(doc) in E.R. at that moment. You have to follow the Triage to decide whom to attend first and your choice can sometimes cause riots in the E.R. if all those patients have their relatives alongside them. Securing IV lines for all of them, especially obese fellows can be a nightmare if you are new to the job. Even when you are done with the examination and treatment part, you are still left with writing notes for all of them and you pray that no more patients should arrive at that time.

You will often get to see M.I. (Myocardial Infarction), Stroke or CVA, Electrocutions, Burns, Teen Suicides, Drug Addicts, Domestic Fights, Dog Bites, Rat Bites etc.

One of your jobs will be to make Medico Legal Certificates (MLC) for Medico-legal cases where informing the police is essential. Sometimes Police bring up local goons for Medical Examination before putting them in Jail. The Goons are often brought together in hordes at night time, which fills up the E.R. and your night can get ruined making MLCs.

Sometimes, there would be an apprehensive woman, who would only want to be treated by a 'Lady Doc' (as they say) and then create a ruckus in the ER room only a Sigma female doctor can manage them as non-sigma gets abused and called too naive to check her.

There would also be occasional 'Brought Dead' or almost on the verge of extinction cases, where you may try everything with C.P.R. to intravenous drugs. In such cases, you would want to have a good Nurse who can support you well and most importantly have a Tough Guard who can handle the E.R. door and won't let other patients enter at that moment. Once again, you have to fill up the paperwork, make a Death Certificate, and send the body to the Mortuary for Forensic Analysis.

Basically, as an E.R. doctor, you need to be a bit commanding, if you have to get any work done. Your Nurses, your assistants, and your patients, all be older than you in age and can sometimes get on your nerves but you need to maintain your calm, demeanour and patience. The work is a tad bit difficult initially, but as they say, you learn on the job.....

Tista: How you are still alive?

Priya: I even started to spend nights in the doctor's chambers but I need to go out of the hospital to remind myself hospital is not my home.

Priyanshi: My brother considers the Hospital his home. He rarely comes home when he misses home food or his room.

Tista: Doctor Arihant is a workaholic?

Priyanshi: Yeah.

Priya: I am not sure when I can go on shopping with you people. I'm not even sure if I will get any time after lunch.

Priyanshi: Awwe and you are spending your lunch with us on the place of resting.

Tista: Now I pity you.

Priyanshi: You have got so many dark circles.

Tista: Looks like a panda.

Priya: Is it a compliment or an insult?

Tista: Both I guess. Hahahaha

Priya: Pffhahahaha

Priyanshi: Hahahaha

Priya sights

Priya: It's kind of refreshing to talk with my close friends.

Priyanshi: Yeah it is.

Priya: I feel like it's the age where I feel so unstable.

Tista: Heck me too, so much to worry about and to do.

Priyanshi: Ah too much to think about what to do.

Priya: I even think that what I'm doing is right or not. I...feel like leaving everything and running away from here.

Tista: I will come with you too.

Priyanshi: Hey, take me with you too. I don't wanna be left alone here.

Priya: Okay.

Priyanshi: You won't leave us alone right?

Priya: I. (Can I make a promise which I can't fulfil?) I-

Suddenly Priya's phone rings.

Priya: Oh. I need to go it's from the ER.

Priyanshi: But you haven't eaten full!

Priya: I will have a sandwich later. See you guys later.

Tista: Yeah.

After Priya leaves

Priyanshi: It's quite heartbreaking to see how she is never at peace.

Tista: Yeah, I do remember she quit acting so she could finally spend time like us but she was often surrounded by a large number of people group. In past our seniors often picked on her on purpose and created scenes where families had to be involved to solve it and because of it, she came on the edge with her aunt where she could no longer have happy or at least normal family time.

Priyanshi: Her Aunt is still living in the old 60s when parents used violence to discipline the kids.

Tista: She does not deserve to be with her daughter. She was not able to be a good mother because of her desire and narrow-minded thinking.

Priyanshi: We were never a help. All we did was act along with her when she tried to hide that her Aunt punished her.

Tista: That's all we could do after all.

Priyanshi: But I wish for a change.
