Kidney donation is one solution to help treat patients with kidney failure. While noble and can save lives, these actions are not without risk. Side effects of kidney transplant include pain, infection in the surgical area, pneumonia, hernia, blood loss and the most fatal is death.

The first thing that needs to be done when you want to become a kidney donor is to assess the compatibility of your kidney with a potential kidney recipient. This examination will be carried out by a medical team at the hospital who will perform kidney donor surgery.

Kidney compatibility can be determined from blood tests, both blood type tests, blood cross tests, and if needed, HLA tests to determine the match between recipient and donor kidney stem cells. Blood type examination is carried out using the A, B, AB and O system. The recipient and donor must have the same or compatible blood group, namely: If the recipient has Blood Type A, then the Donor blood group must be A or O If the recipient has Blood Type B, the donor's blood type must be B or O If the recipient has O blood type, then the donor's blood group must be O If the recipient has AB blood type, then the donor's blood group can come from all blood groups (A, B, AB or O If the recipient receives an inappropriate kidney donor, the recipient's body will recognize the kidney as a foreign object and damage it.

The next test is the examination of Human Leukocyte Antigen (HLA) from the blood to identify the types of tissue types from both the donor and the recipient. Antigens as markers can be found in various body cells that distinguish each person uniquely. This marker is passed down from both parents.

To be able to receive a given kidney, the recipient marker and donor marker must have a high level of similarity (perfect match). Transplants with a high degree of similarity will give better results over the next few years. A good match rate is usually found in donors who are still close relatives of the recipient.

Antibodies will be formed at the time of infection, pregnancy, blood transfusion, until the transplant process. If there are antibodies against the donor kidney, the recipient's body will destroy the kidney. Therefore, a cross-match must be carried out to ensure that the recipient can receive the donor kidney properly. The test is done by mixing blood from the recipient with tissue cells from the donor.

If the cross test is positive, it means that the recipient has antibodies against the donor. On the other hand, if the cross-test result is negative, it means that the recipient does not have antibodies against the donor and can receive the donor's kidney.

This cross-test is performed several times during the transplant preparation stage, and the final test must be performed within 48 hours before the transplant is performed. Serological examination was carried out to determine the presence of HIV, hepatitis, and cytomegalovirus (CMV) virus infection so that appropriate preventive therapy could be given after transplantation.

This examination is carried out on all donors, in order to prevent transmission to the recipient.

However, people with diabetes and hypertension are still able to donate their kidneys, if their blood sugar levels and blood pressure are well controlled. 

A kidney donor should also not suffer from other diseases, such as cardiovascular disease, lung disease, blood clotting disorders, cancer, and HIV/AIDS. Before a kidney donor is carried out, the doctor will conduct a thorough medical examination to ensure that this procedure will not endanger the health of the donor and recipient.

If the results of the examination state that the donor kidney is healthy and matches the recipient's kidney donor, and the donor's overall health condition is good, kidney transplant surgery can be performed. Ayu listened to Satya's explanation very carefully and without interrupting at all.
