After the Kara mess and destroying Jigen and saving Kawaki, Boruto and others learnt the real threat of Karma, a four pointed diamond shaped seal, bestowed by Momoshiki Otsutsuki. The resurrection of Momoshiki is 80% complete and Boruto loses his sleep and hunger in fear of Momoshiki taking over his body. Soon, Boruto overheard the Kages talking about killing him to save the world. After hearing that, Boruto convinced himself that him dying is the only way to save the world. (The kara doctor doesn't have the pills in my story.) Without any sleep or properly taking care of himself, Boruto slowly slipped into multiple mental and physical illnesses, eventually attempting to kill himself. He was saved by Tsunade and Sakura.
