Wakanda has separate dimension where soul of who died goes.though it's not clearly told I will change it that those who took heart shaped herb, it changes more than the physical body , may be at spiritual level.
It stimulates and allow them to access to astral travel like in doctor strange , and due to their belief and connection in bast they travel to ancestral plane and meet past panthers. As for N'jobu appearance, I will say it's may be due to that in/with ancestral plane we can also see spirits of others who are not related to herb unless they are sealed or alive or some special circumstances.
