though puzzled, the passerby would half-heartedly have no resistance and, whether or not they believe it, most people would go to the hospital for a check-up.
But if it were a psychiatrist who suddenly said to a passerby, "You have a mental illness, I can help you,"
the passerby would only think the doctor is the one with a mental illness.
