To describe it, Kiyo was referring to Meryna's ability that's called 'Mind Of The Pink Puppet'. An ability that led Rio to survive even though all four of their limbs were decapitated from his body. In exchange, Rio will become a doll with no way to control any part of his body.
The Monoscope shows Rio's entire body part that's been controlled by some pink line, mostly coming from his nerve. Kiyo's first instinct was to destroy the source of it, the forehead.
