This was the scene Minerva saw after the explosion.
It seemed they had been hit by a thrown bomb or shell; the hastily raised Barrier managed to deflect most of the impact, but they were still thrown out.

She later learned that there wasn't just one bomb. The first explosion had ignited other bombs, resulting in a second, even more powerful blast.
This time, she didn't even have the strength to raise the Barrier, and she was directly blown into the bottom of the valley. Amidst the pain and impact, she lost consciousness and fell into the water, swept away by the Rhine River, and her memory stopped there.
When she regained consciousness, she and Roland had already been washed up on the riverbank, their clothes hanging off their bodies like tattered rags, with Roland still unconscious.
