The nurse didn't waste any time and pulled out a submachine gun, and pulled the trigger.
"Joker!"
Off-screen, both players and many viewers immediately recognized that the nurse with bizarre makeup was actually Joker in disguise. This lunatic was really planning to blow up the hospital!

"Okay, you and Rachel were kidnapped while I was locked up in Gordon's prison. I wasn't the one behind this plan!"
Harvey Dent, of course, wasn't fooled by Joker's feeble attempts at deception and said coldly, "You provided the plan and the manpower!"
Joker calmly stated, "Do you think I'm the kind of person who follows a plan? You don't really understand me. I'm like a dog chasing cars. Even if I catch one, I wouldn't know what to do with it. You see, I'm just doing what I do for the sake of doing it... The police have plans, the gangsters have plans, and even Gordon has plans. They are all conspirators. Conspirators always hold their 'little world' in the palm of their hand. I'm not a conspirator; I'm the one who makes those conspirators realize how pathetic and laughable their efforts to control everything are. So when I say your tragedy and your girlfriend's were not personal vendettas, you'll know that I'm speaking the truth."
Indeed, this was Joker's truth. He wasn't targeting Harvey or Rachel out of personal vengeance; he wanted more—chaos!
Joker was the source of all this chaos, but one had to admit that this madman had quite the eloquence. He told Harvey Dent that the reason he had fallen into such a miserable state was due to the conspirators, and Harvey Dent was one of them. His own plans had brought him to this tragic end.
While saying all this, Joker casually untied Harvey Dent's restraints, prompting Harvey Dent to resist, but he was easily subdued.
