Rogue entered the birthing chamber with the poisoned bowl in his hands. Nightshade is odorless and does not turn silver to black. The bowl servants have given him is silver, with wolf emblems. 

Obviously, a Winterchild family heirloom. 

Thus, when Bettie Troll took the bowl, she did not suspect poison at all. She has been a docile girl for the most part, obedient, and hard working. She may be a little more sturdier than what most lords would prefer, thus, she was fairly happy when lord Flayer married her. 

After all, she did not know Flayer history. 

For the fear of ruining the marriage, Walter Troll ordered the caste to keep mum about Flayers. On top of that, there is that innate fear they felt towards lord Flayer, so they kept mum about the details. 

After coming to the north, instead of the skull fort, Bettie entered the strong but cozy strongfort, making her even more unaware of the cruel nature of her husband or his bastard son.
